
SNN is a transformational, VCSE-led proactive care model that has evidenced across 3 years how we can support people with complex needs and prevent people’s needs escalating, with an aim of reducing demand and cost for statutory services.
We remain evidence-based and demonstrate impact of our coproduction and collaborative approach.
SNN has been recognised nationally by being shortlisted for the MJ Awards with North Northamptonshire Council who have co-led this service.
We want to continue to deliver our core service and continue to test arrangements through our other pilots in partnership with
Adult Social Care, the Police, Greatwell Homes and Kettering General Hospital plus others to ensure that we provide early intervention and support while building personal and community resilience.

• Service User Friendly Forum spoke at Health Inequalities and Community Cohesion Conference about their lives and experiences of support, in front of over 100 people.

Things moved quickly and [I was] referred to the correct services and the PIP was reinstated and a big back payment. It all happened so quickly but with SNN by my side I felt comfortable. SNN could do even more than I expected. Whatever I needed was attended to.
