
SNN is a transformational, VCSE-led proactive care model that has evidenced across 3 years how we can support people with complex needs and prevent people’s needs escalating, with an aim of reducing demand and cost for statutory services.
We remain evidence-based and demonstrate impact of our coproduction and collaborative approach.
SNN has been recognised nationally by being shortlisted for the MJ Awards with North Northamptonshire Council who have co-led this service.
We want to continue to deliver our core service and continue to test arrangements through our other pilots in partnership with
Adult Social Care, the Police, Greatwell Homes and Kettering General Hospital plus others to ensure that we provide early intervention and support while building personal and community resilience.

• 668 referrals from 52 different referral sources.
• 81% of people support with high Levels of Need.
• 3.4 areas of support required on average.
• 86 CATCH (Community Action Team Can Help) Co-ordination Meetings held.
• 209 partner services and organisations (48% VCSE).
• 4.8 out of 5 average satisfaction score. • Successful Positive Action programme that increased
• access for Black, LGBTQ+, and Disabled communities.
• Service User Friendly Forum spoke at Health Inequalities and Community Cohesion Conference about their lives and experiences of support, in front of over 100 people.
• Over £399,000 of financial impact for service users
